Mr. Luis As usual your support was appreciated. The quality of welding is seems to be the root cause. Can we inspect the others locations and what is recommended practice to perform this job. Are PT&MT tests will be fine to inspect the welds integrity?

Mark Fillet weld should have been qualified with PQR / WPS in original manufacturing welding procedure / performance qualification tests. If not done, qualify your welders as per ASME Sec IX PQR / WPS needs. DPT of the first run should mostly solve your problem. You can also do MPT of the root run if needed. Final weld - do DPT.
