I'm now conducting root cause analysis (RCA) for a steam service pipe which experienced pinhole at weldment. The flowrate from the discharge pump is at approximately 10BarG. For info, the weldment is near to the bend. Please advice of the root causes of the pinhole leak and the remedy. The pipe material is Carbon Steel.

Hasmid: It's not possible to supply a "Root cause" analysis, based on a simple description of the symptom. The leak could be from a bad weld, from cavitation, etc. You should conduct ulltrasonic wall thicknness measurements around the leak to better understand if the wall loss is local or not.
